Official Notice for Newly Accepted Master's Students (2023-2024)
Accepted students are required to visit the college to complete Guarantee and Study Leave procedures. Please bring two color copies of the University Admission Order.
Required Documentation (Student & Guarantor):
National ID or (Civil Status ID + Nationality Certificate) - Original + Color Copy.
Residence Card - Original + Color Copy.
Two (2) recent personal photographs for both parties.
Support of Service Continuation addressed to the 'College of Pharmacy'.
Legal Notice: Personal presence of both the Student and the Guarantor is mandatory.
